July 20, 2011 Houston, Tx.: On October 3, 2007 a crew member aboard the M/V Galena Bay ship tripped on chains improperly stored on a ship's deck.

The injured worker claimed it was only after his fall that welders boarded the vessel and made necessary repairs to the chains so they could be safely connected to posts storied off the deck.
